Oriented x 3
Explanation
Oriented x3 usually means that a person correctly identifies who they are, where they are, and the time or date. The number three refers to these areas of orientation. It does not mean that every thinking ability has been tested.
Does this rule out memory problems?
No. Remembering new information and paying attention to a task need separate assessment. The x3 note alone does not tell you whether those abilities were tested or what the results were.
How does it differ from x4?
The x4 wording usually adds awareness of the situation, such as why the person is receiving care. An x3 note alone does not tell you whether that question was asked. The full examination record gives more detail.
